Application form for prospective membership
The registration form serves to assess whether you qualify for the Student Membership. If, after reading the admission requirements, you believe you meet the criteria for NVA membership, we ask you to fill out the registration form and send it to the secretariat.
Important information to prepare for Practicing Membership
In addition to needing a copy of your acupuncture and/or western medical diploma, or a provisional proof of passing, when transferring to Practicing Membership, you must meet several other requirements.
